Classroom Forms
These forms are necessary to successfully conduct and complete an NAU Distance
Learning class.
Desk Copy Request Form
used to obtain a desk copy of a textbook from the publisher.
Grade submission form:
used to record and submit final class grades used to record and submit final
class grades.
Incomplete Contract:
Complete this form with a student before assigning the grade of "Incomplete."
Procedures and policies regarding Incomplete grades can be found on the form or
in the Distance Learning Faculty Handbook.
Travel Expense Record Form (.pdf):
submit for reimbursement of class-related travel expenses.
Field Trip Release Form: Faculty should have students complete this form prior
to a field trip or class meeting held outside of the normally scheduled
classroom. The forms can be returned to the Local Campus Office to be sent to
the main NAU campus.
